Child Care Checklist
.	1  Does the provider have a current license posted or is it available when you ask for it? What is the license capacity? Does she have more children present than the capacity on the license allows?
.	2.  Does the provider have the following posted where all parents can see: Her CPR/First Aid card If she has assistants, their CPR/First Aid cards the "Parents Rights" Poster"
.	3  Does the provider have copies of all visits from the licensing agency for the past 3 years? A copy of the licensing visit must be posted for 30 days after the visit, and then kept readily available for parents to view for the past 3 years.  You can call Licensing to receive information on the provider by calling 408-277-1286 and ask for the Family Child Care Division.
.	4  Does the provider have a disaster plan and is it posted?  Does the provider have regular Fire and Earthquake Drills? Do the children know where to meet outdoors in case of an emergency?
.	5  Are references available? Is the assistant ever left in charge of the children while the provider is gone?
.	6  Are you allowed to make unannounced visits to observe the program? (By law you are able to enter the home unannounced any time during the period of time when your child is in attendance.)
.	7  Does the home seem safe? Are lights and sockets covered or out of reach? Is the climbing equipment safe? Are harmful plants, poisons, and medicines stored out of reach? Can the children get inside and outside safely and without difficulty?
.	8  Is the equipment inside and out varied, sturdy, and easy for a child to use? Is any of it broken? Is there adequate space for active and quiet play as well as a designated sleeping area?
.	9  Are nutritionally balanced meals furnished by the provider?
.	10  Does the discipline policy exclude any form of corporal punishment and include positive redirection?
.	11 Does the provider have provision for an ill child? What is the sick child policy?
.	12  Is the home attractive and comfortable? Are there special areas for activities? Is the home arranged easily for learning and growing?
.	13  Are the materials ample, in good condition, and easily available? Can children move around and reach a variety of materials easily? Are the learning opportunities suitable for different ages?
.	14  Do the children show they really like and trust the adults a lot? Do they receive the individual respect, attention, and assistance they need? Are their questions answered easily?
.	15  Do the children appear happy, comfortable, and relaxed? Do they respond easily and happily to each other? To the adults? Do they enjoy the activities available?
.	16  Are special events and trips usual activities?
.	17  Are problems handled calmly, sensitively, and serenely?
.	18  Does the provider answer your questions directly and openly? Does she clearly communicate her expectations? Is there a written agreement or policy sheet?
.	19  Do I feel comfortable with the provider, the other children, and with the home?
.	20  Does your child feel comfortable with the provider, the other children, and with the home?

.	11  ¿El proveedor tiene un plan preparado para cuando un niño(a) se enferma, cual es la politica a seguir en esta situacion?
.	12  ¿Es el hogar atractivo y comodo? ¿Hay algunas areas especiales para actividades? ¿Esta el hogar arreglado de una forma que facilita la enseñanza y el crecimiento de los niños?
.	13  ¿Tiene suficientes materiales en buena condicion y al alcanse de los niños? ¿Se pueden mover los niños y obtener facilmente materiales de diferente variedad? ¿Las oportunidades de aprendizaje estan dirigidas para niños de diferemtes edades?
.	14  ¿Los niños demuestran que realmente les gustan y confian en los adultos? ¿Los niños reciben respeto, atencion y ayuda en lo que necesitan? ¿Son contestadas sus preguntas sencillamente?
.	15  ¿Los niños parecen felices comodos y relajados? ¿Hay una comunicacion feliz entre ellos y con los adultos? ¿Disfrutan las actividades disponibles?
.	16  ¿Tienen eventos especiales y acostumbran actividades de paseo?
.	17  ¿Los problemas se tratan calmadamente y de una manera sensitiva y serena?
.	18  ¿El proveedor(a) contesta sus preguntas de una manera directa y abierta? ¿Tiene una comunicacion clara en sus expectativas? ¿Existe un contrado por escrito o una hoja de politicas?
.	19  ¿Se sintio usted comodo con el proveedor, con otros niños y con el lugar?
.	20  ¿Su niño se siente comodo con el proveedor con otros niños y con el lugar?
